3.  GRADES & PROGRESS REPORTS (Report Cards, quarterly reports, etc.)

4.  STANDARDIZED TEST RESULTS (Stanford 9, Star Tests, etc.)

5.  DISCIPLINE RECORDS

6.  DECISIONS OR SETTLEMENTS IN ANY PRIOR DUE  PROCESS HEARINGS

  • PLEASE DO NOT GIVE US ORIGINAL DOCUMENTS 

  • Make clear, legible, one-sided copies.

  • Group the documents according to the categories listed above with the most recent documents on top.  

  • Place a rubber band around each separate category of documents; do not put the documents in binders. 

  • Please indicate if you do not have any documents for one or more of the categories.
